Incompetently written IEP's-out of compliance with IDEA regulations as well as consistent procedural and substantive violations of FAPE under IDEA 2004-including but not limited to: omitting IEP invitations to school personnel legally mandated to attend IEP team meetings, failure to notify parents of IEP meetings,failure to hold IEP team meetings within legally required time lines, failure to conduct proper assessment to determine needs and services during triennial re-evaluations.

Reports of hitting students with disabilities upside the head and on their back with an open hand

Report of calling student with severe disability a "retard"

Report of making female student with disabilities cry in class when "providing instruction"
